Please note as of August 26, 2026- we are not currently accepting patients at our Ingersoll location.
Registration is open. If you would like to become a patient, please complete the registration form using the link below. Please have any relevant medical information available, including your current medications, medical conditions, surgeries, allergies, vaccination history, and the contact information for your previous healthcare providers. Submitting a registration form does not guarantee enrollment, but all applications will be reviewed according to clinic eligibility criteria and available capacity.
Do you live in Woodstock? Please note, we are not currently accepting or prioritizing new patients who live in Woodstock. To support coordinated regional care, Woodstock residents are encouraged to contact the Thames Valley Family Health Team. The Thames Valley Family Health Team is accepting new patients at 959 Dundas Street, Woodstock. Patients registering there will be attached to a nurse practitioner.
